When receiving medical treatment at a healthcare facility outside of Kanagawa Prefecture, please apply for reimbursement after personally paying the medical expenses to the healthcare facility. The application should be submitted from the month following the month of the medical examination onwards.
1 Required items
a) Medical care card for children
b) Passbooks of financial institutions or proof of account number(The applicant's name as it appears on the medical care card. Otherwise, g) a Power of Attorney is required)
c) Receipt from medical institution (showing name of recipient, date of treatment, name of medical institution, and co-payment for insured medical expenses)
d) Notice of decision to pay high-cost medical care and additional family medical care benefits, etc. (only for those who apply)
e) Your child's health insurance card
f) For refunds for medical orthotics, etc., documents that can confirm the date of medical treatment (e.g., a copy of the medical certificate)
g) Power of Attorney (Only if you wish to make the transfer to an account other than the one in the applicant's name as shown on the medical care card. 2 Application Period
Five years from the following month of the medical examination (e.g., for the examination conducted on April 1, 2019, the application must be submitted by April 30, 2024).
Please note that in order to make calculations after confirming whether or not the medical expenses for the month in question are covered by other systems such as high-cost medical care, etc., we ask that applications be submitted in the month following the month of the medical examination.
If five years have passed since the medical examination, the prescription will expire and the subsidy will not be available.

The Elderly, Long-Term Care and Medical Expense Subsidy Section of the Insurance and Pension Division of each ward office or the Insurance and Pension Section of each Citizens' Center.
Applications by mail are also accepted. Please refer to the link below for details as the documents required by mail are different.
