If you want to re-issue National Health Insurance, please take the procedure below.

1 Where to request:

Insurance and Pension Division of ward office where you live, Branch Citizen Center.

2 How to request:

Go to the desk directly

3 Reception Hours:

Mon-Fri 8:30 AM to 5:00 PM (ward office only)

2nd and 4th Saturdays 8:30 AM to 0:30 PM

4 Holiday:

Saturday, Sunday, National Holiday, 29 December-3rd January

(Ward office is available on 2nd and 4th Saturday)

5 Required Items for Notification:

(1) Seal

(2) My Number Card (Individual Number Card) or a Notification Card with a written name and address that matches on the certificate of Items Stated in Resident Register.

(3) One Document can be confirmed the applicant’s name. (Driver’s license/Passport) or two documents which issued/provide by government office.

* In case of loss/theft, if you are concerned about misuse of your insurance card, you can request to disable the card, so please file for it.

* If you found your insurance card after you reissued, please return the old one.

* If the person who want to apply, and bring the documents verifying your identity with face photo, the insurance card can be issued at the counter. If you don’t have the document verifying identity, we appreciate your understanding that the insurance card will be sent by mail at a later date to confirm your identity.

*If a non-family member who is in the same household as the person who lost the insurance card files a notification, a Letter of attorney and the document verifying identity that the person who visit the counter are required.
