A moving-in notification is required if moving from outside of the city.
1. Documents to be Submitted: Moving-In Notification (Notification of Resident Change)
*You can obtain the form at Ward Residents Section of ward office / Citizens' Centers of branch offices.
2. Submission Deadline: Within 14 Days after Moving-in
*Note that you cannot submit your notification if you have not finished moving.
*Note that, according to the Basic Resident Registration Act, you may be penalized up to 50,000 yen if you are late for submitting notification.
*Note that foreign residents living for a medium-to-long term in Japan based on their qualifications as decided by immigration control and Refugee Recognition Act may lose their residential qualifications if they do not submit notification of their residence without adequate reason.
3. Where to Submit Documents: Ward Residents Section of ward office / Citizens’ Center at branch offices of your domicile
4. Notifier: A person him/her selves or a householder
* In case of that a representative submits notification, the following documents are required.
(1) In the case that a legal representative (parents, a guardian of a minor, a guardian of an adult, etc.) submits: A family register or other documents that prove his/her qualification as a legal representative
(2) In the case that any person other than a legal representative submits on behalf of him/her: A letter of attorney
5. Submission Procedure: Submit them directly at a submission counter.
6. Office hours: Monday - Friday, 8:30 am-5:00 pm
The 2nd and 4th Saturdays, 8:30 am-0:30 pm (Citizens’ Centers of branch offices are closed for the reception on the 2nd and 4th Saturdays.)
7. Holidays: Saturdays (excluding the 2nd and 4th Saturdays), Sundays, National holidays, Dec 29th - Jan 3rd (Citizens' Centers of branch offices are closed on the 2nd and 4th Saturdays also.)
8. Required Items:
(1) The certificate of moving-out (It is issued at the local office of which you lived in.)
(2) Any of the following identity verification documents:
a) Basic Resident Registration Card with face photo / Individual Number Card / Any License, Permission or Certificate of Qualification which is issued by public agencies such as Passport or Driver's License (with a face photo attached only)
b) Any two documents that confirm your name: Health Insurance Card / Cash Cards / Bankbook etc.
(3) Individual Number Card (If you forget to bring it, please bring it later.)
(4) Basic Resident Registration Card (Only those who hold it)
(5) A certificate of recipient qualification (those who are authorized to receive for long-term care at the previous domicile)
(6) A copayment category certificate (those who hold a medical care certificate at the previous domicile)
(7) In case of that a representative submits notification, the following documents are required.
a) In the case that a legal representative (parents, a guardian of a minor, a guardian of an adult, etc.) submits: A family register or other documents that prove his/her qualification as a legal representative
b) In the case that any person other than a legal representative submits on behalf of him/her: A letter of attorney
(8) For foreign residents, please bring the Resident Cards or Special Permanent Resident certificates for all the family members who move in Kawasaki addition of the items above. If we cannot confirm your qualification and period of stay, you should bring your Passport or Certificate of Eligibility.
(9) An unregistered seal (It might be needed for procedures except the Moving in notification.)
9. Notes
(1) Note that you cannot submit notification in person or by mail if you have not finished moving.
(2) While it may be possible to issue a certified copy of your Residence Registry on the same day that your notification is received, it may require additional time depending on the how busy the counter is.

(4) As a special exception, Moving-in procedures for those with the Basic Resident Register Card or My Number Card are accepted on weekdays (Monday - Friday, 9:00 am-5:00 pm, nationwide).
