If your doctor or midwife has diagnosed you as pregnant, you should submit a "Pregnancy Notification" (including the "OTAZUNE" on the back) to the Community Care and Support Center or the Community Health and Social Welfare Station in your domicile.

8:30 AM-5:15 PM
If the representative submits the notification, the letter of attorney is required.
For the necessary documents or application counter, please confirm related page “Pregnancy Notification and Mother and Child Health Handbook”.
Note
For foreigners who is difficult to understand Japanese, we can issue Mother and Child Health Handbook in 10 languages [English, Chinese, Hangul, Spanish, Portuguese, Indonesian, Tagalog, Thai, Vietnamese, Nepali] instead of the Japanese version.
If you move in Kawasaki during pregnancy or after childbirth, you can still use the Mother and Child Health Handbook, but please visit the Community Care and Support Center or Community Health and Social Welfare Station in your ward since we will issue the coupon for prenatal checkups and provide an explanation for health service.
If you lost your Mother and Child Health Handbook, please also visit the Community Care and Support Center or Community Health and Social Welfare Station in your ward.
