When a foreigner enrolls or withdraws from the National Health Insurance system due to employment or retirement, the procedures are as follows. Please note that the items required for notification may differ depending on the situation.
1. Submission Period: In principle, within 14 days of circumstances occurring.
2. Inquiry Counter:
(1) If you do not take the procedure such as moving-in or moving-out.
Insurance and Pension Division of ward office or Insurance and Pension Desk, Citizens Center of Branch office in your area.
(2) If you take the procedure such as moving-in or moving-out.
Ward Residents Division of ward office or Resident Record/Child allowance/School Desk, Citizens Center of Branch office in your area.
3. Submission Method: Submit directly at the Counter
4. Office hours: Monday to Friday 8:30 am - 5:00 pm
2nd and 4th Saturdays 8:30 am - 0:30 pm (Ward Offices Only)
5. Holidays: Saturdays, Sundays, National holidays, and December 29th to January 3rd (Ward offices are open in the mornings on the 2nd and 4th Saturdays).
6. Required Items for Submission
(1) For foreign residents enrolling National Health Insurance
A. Resident Card
B. Passport (Cases where the status of residence is "Designated Activities")
C. My Number Card (Individual card) or Notification Card (Only those who hold it.)
D. Certificate of Loss of Qualification for Health Insurance of your work place (Shikaku Soshitsu Shomeisho)
E. Student ID, etc.
F. Kanagawa Prefecture Resident National Health Insurance Card, if you have already entered National Health Insurance within the same household.
(2) For Foreign Residents Withdrawing from National Health Insurance
A. Resident Card
B. My Number Card (Individual card) or Notification Card (for householder or the person who withdraw National Health Insurance.)
C. Health Insurance Card of your work place or Health Insurance Qualification Certificate
D. Kanagawa Prefecture National Health Insurance Card
*If the person submitting the notification is not the applicant withdrawing, or is not a member of the same family, a letter of attorney and documents verifying your identity are required at the counter.
7. Eligibility Requirements: A foreign person who is a registered resident of the city (those who have resided here for more than three months since their residential status has been approved). Even if the period is under three months, those who have a resident status in “Entertainer”, “Technical Intern Training”, “Dependent”, “Designated Activities”, and individuals who are permitted to stay in Japan for more than three months (with provable documents) are eligible to apply.
You may not apply if your purpose for residing in Japan is “To receive medical care”.
You do not need to enter to National Health Insurance if you have been issued a “Certificate of Applicability” for social security agreements from the USA, Belgium, France, the Netherlands, Czech Republic, Switzerland, Hungary, and Luxembourg.
