It is not possible to use Kawasaki City government service terminals if you have forgotten your password or the password lock has been activated because of 5 incorrect password attempts. Please re-register your password by making an application at the ward office or branch office with jurisdiction over your residence. Procedures are as follows.

(1) Necessary documents: Application for re-registration of user identification card password (available at corresponding counters）
(2) Counters: Ward office resident sections and resident centers in your locality
(3) How to apply: Apply directly at counter
(4) Items required: Kawasaki Citizens' Card, personal seal (informal/unregistered seals are accepted), power of attorney for applications through a proxy
(5) Charges: Free
(6) Business hours: 8:30am - 5:00pm, Monday - Friday
8:30am - 12:30pm, 2nd & 4th Saturday of the month
(Residents centers are not open on Saturdays.）
(7) Closed: Saturdays (except 2nd & 4th Sat.), Sundays, national holidays, December 29 - January 3

Same-day password re-registration is possible if a driver's license, passport, Basic Resident Registration Card (only cards displaying photos) etc. or some form of identification (with photo) issued by a government office is provided.
If persons do not have some form of identification (with photo) issued by a government office, or otherwise the application is submitted by a proxy, an inquiry form will be forwarded to the applicant's (actual person concerned) address in order to verify consent. Following receipt of the inquiry form, the applicant (actual person concerned) should complete and bring the form to the applicable counter together with documentation supporting the name displayed on one's health insurance card. Passwords can only be re-registered upon completion of the above.
(Several days are required to process the application.）
The actual person concerned (applicant) is required to go to the corresponding counter to re-register the password. (A proxy is only able to submit an application.)
