If you have entered into insurance through your workplace, you can withdraw from national health insurance through the following procedure.

1. Submission Period: In Principle, within 14 days of the reason occurring.

2. Submission Counter:

(1) If you do not take the procedure such as moving-in or moving-out.

Insurance and Pension Division of ward office or Insurance and Pension Desk, Citizens Center of Branch office in your area.

(2) If you take the procedure such as moving-in or moving-out.

Ward Residents Division of ward office or Resident Record/Child allowance/School Desk, Citizens Center of Branch office in your area.

3. Submission Method: Directly at the counter, or apply electronically through the internet

4: Reception Time: Monday-Friday 8:30 a.m. - 5:00 p.m.

Second and fourth Saturdays: 8:30 a.m. - 0:30 p.m. (Ward Offices Only)

*Internet applications are accepted 24 hours a day (in principle).

5. Closed: Saturday, Sunday, holidays, and December 29 - January 3 (Ward offices are open in the mornings on the second and fourth Saturday.)

6. Required Items

(1) Kanagawa Prefecture National Health Insurance Card

(2) Workplace Health Insurance Card or Health Insurance Qualification Acquisition Certificate

(3) Seal

(4) My Number Card (Individual Number Card) or Notification Card with the same name, address, etc. as the Items Sated in Resident Register. (For Head of Household and for Individuals Withdrawing from National Health Insurance)

(5) Documents Verifying the Identity of the Applicant (1 Photo ID (Driver’s License or Passport, or 2 Documents Issued/Provided by the Government)
