I would like to know about the procedure to use long-term care insurance services.

The procedure to receive the long-term care insurance service is as follows.

1 How to take a procedure

(1) To receive nursing care services, apply for a Care-Needs Assessment, etc. at the counter of Community Health and Social Welfare Station in the ward office to be certified as Care-Needs Assessment or Assistance-Needs Assessment.

(2) If you are certified as Level 1~5 of Care-Needs Assessment, you will choose a designated home care support service operator and ask a care manager to make plans for the nursing care that you will be taking ("care plan"). If your level of Assistance-Needs Assessment is determined as 1 or 2, contact the Community Comprehensive Support Center in your area and ask health nurses to make a preventive long-term care service plan or make a preventive Care Management. (You can prepare the care plan by yourself also. When you use a care service facility, such facility will prepare the plan for you.)

2 Eligible Person

(1) Those who are over 65 years old and have a domicile in Kawasaki City. (The First Insured Person)

(2) Those who are over 40 years old and under 65 years old, and have a domicile in Kawasaki City having a health insurance (the Second Insured Person)

3 Office hours: Monday to Friday  8:30am to 5:00pm

4 Holidays: Saturdays, Sundays, National holidays, Dec.29 to Jan.3

5 Service Contents: In nursing care services (for those who are certified as level 1 to 5 of Care-Needs Assessment), there are home care services, nursing care services at facilities, and community-based nursing care services. In preventive long-term care services (for those who are level 1 to 2 of Assistance-Needs Assessment), there are home care services and community-based services for prevent long-term care. Generally, 10% or 30% of care cost will be borne by the care recipient when using each service.

*When the care recipient or his/her family is not able to apply to be certified as a Care-Needs Assessment (Assistance-Needs Assessment), a designated home care support service operator, a facility covered by long-term care insurance, or the Community Comprehensive Support Center can apply for you.
