Answer

During Office hours, a Family Register Notifications are accepted at the Ward Residents Section of Ward Offices and Citizens' Center of Branch Offices in your domicile or in the ward where you have your registered address or you are staying temporarily. Outside of Office hours, a Family Register Notifications are accepted at a security guard stations of ward office however, the documents are not reviewed. If your notification is lacking any necessary information, you will be contacted by the person in charge when the office opens. In such a case, the date of notification may eventually change. (Outside Office hours, it cannot be accepted at the Citizens' Center of Branch Office)

Regarding the Request for Non-acceptance, it cannot be accepted at a security guard stations of ward office. Please submit to the Ward Residents Section of Ward Offices and Citizens' Center of Branch Offices in your domicile or in the ward where you have your registered address or you are staying temporarily, during the business hours.

1. Document to submit: Notification of birth, death, marriage, divorce, and so forth for registration in family registers. For more information on notifications, please see the related website: Family Register Notification (Notification of birth, marriage, etc.).

2. Where to notify: Security room of Ward Office

3. Notifier: The person him/herself (Relatives can submit notification if the person him/herself has completed the document. [A letter of proxy is not required.] However, the person him/herself may be required to visit the office another date.)

4. Office hours: Outside Office hours (early morning, late night, and over ward office holidays)

5. Notes: You will be contacted by the Section in charge (Family Resister Desk, Ward Residence Section) another date if your application is lacking any information or attached documents required. Please fill in a daytime contact telephone number on the notification form.

Monday – Friday, 8:30am to 5:00pm

2nd & 4th Saturdays, 8:30am to 0:30pm (at the ward office only)
