Income tax return forms are available at National Tax Agency offices, and they can also be downloaded from the National Tax Agency homepage.
Also on the National Tax Agency homepage there is a “Corner for Making Tax Returns etc.” (確定申告書等作成コーナー) where tax returns can be made and entry fields in accordance with their guidance screen for things such as monetary values can be filled in. If you print it out, it can be submitted to the National Tax Agency as is. Please refer to the “National Tax Agency homepage” in the “Related Pages” box below

Also tax return forms will be available at the Municipal Taxation Office, Municipal Inhabitants Tax Section (Municipal Residence Tax Branch Offices, Citizen Tax Staff). The forms are in limited supply during the distribution period, so please inquire about the amount left beforehand at distributing desks.
